Our client, a leading organization in the community and social care sector, is seeking a dedicated and experienced Senior Community Health Nurse to join their fully remote team. This vital role involves providing essential health education, support, and guidance to individuals and families within their communities, leveraging technology to reach a wider audience. You will play a key role in developing and delivering remote health programs, managing patient care plans, and collaborating with healthcare providers.

Responsibilities: Conduct remote health assessments and screenings using telehealth platforms and other digital tools. Develop and implement personalized care plans for patients, focusing on chronic disease management, preventative care, and health promotion. Provide health education and counseling to individuals, families, and community groups on various health topics (e.g., nutrition, hygiene, disease prevention, mental health). Monitor patient progress remotely, track vital signs, and manage medication adherence. Collaborate with physicians, specialists, social workers, and other healthcare professionals to coordinate patient care. Facilitate virtual support groups and educational workshops. Advocate for patients' health needs and connect them with community resources and services. Maintain accurate and confidential patient records in electronic health record (EHR) systems. Stay up-to-date with the latest advancements in nursing practice, public health, and telehealth technologies. Participate in community health initiatives and contribute to program development. Provide guidance and mentorship to junior nurses or community health workers. Educate patients and families on the effective use of medical devices and monitoring equipment. Develop protocols and best practices for remote patient care delivery. Analyze health data to identify community health trends and needs. Ensure compliance with all relevant healthcare regulations and ethical standards. Respond to patient inquiries and provide timely support via secure communication channels. Qualifications: Registered Nurse (RN) license in good standing. Bachelor of Science in Nursing (BSN) required; Master's degree in Public Health or Community Health Nursing is a plus. Minimum of 5-7 years of progressive experience in nursing, with a significant focus on community health, public health, or primary care. Proven experience in telehealth, remote patient monitoring, or case management. Excellent clinical assessment, critical thinking, and problem-solving skills. Strong teaching, counseling, and motivational interviewing skills. Proficiency in using EHR systems, telehealth platforms (e.g., Zoom, Doxy.me), and other digital health tools. Exceptional communication, interpersonal, and active listening skills. Ability to work independently, manage a caseload remotely, and prioritize tasks effectively. Deep understanding of public health principles and community health needs. Commitment to patient-centered care and health equity. Proficiency in Microsoft Office Suite and electronic communication tools. Ability to adapt to new technologies and evolving healthcare landscapes.
